Description: PGCSC contracts out to other providers who provide adaptations (like ramps or grab bars), structural repairs and/or modifications to the client’s home for the purpose of maintaining safety and independence.Eligibility: To be eligible, you must be:
- At least 60 years of age, or
- Disabled, 18-59 years old, and have a physician-diagnosed physical or mental impairment that substantially limits your ability to perform activities of daily living.
